I acknowledge by signing this agreement that I have been given the full opportunity to ask any and all questions which I might have about the obtaining of a tattoo and that all of my questions have been answered to my full satisfaction.
I acknowledge I am over the age of eighteen and that I have truthfully represented to my tattooer that the obtaining of a tattoo is by my choice alone.
I acknowledge that a tattoo is a permanent change to my appearance and that no representations have been made to me as to the ability to later change or remove my tattoo. To my knowledge, I do not have a physical, mental or medical impairment or disability which might affect my well being as a direct or indirect result of my decision to have a tattoo
I do not have medical conditions such as but not limited to: diabetes, epilepsy, hemophilia, a heart condition, nor do I take blood thinning medication. I do not have any other medical or skin condition that may interfere with the application or healing of the tattoo. I am not the recipient of an organ or bone marrow transplant or, if I am, I have taken the prescribed preventive regimen of antibiotics that is required by my doctor in advance of any invasive procedure such as tattooing. I am not pregnant or nursing. I am not infected with any bloodborne pathogens (i.e. hepatitis, h.i.v.)
I have not consumed any drugs (prescription, or non prescription) or alcohol in the past 24 hours.
I have considered and accepted any consequences that may arise from my decision to receive this tattoo.
I understand that if I have any skin treatments, laser hair removal, plastic surgery or other skin altering procedures, it may result in adverse changes to my tattoo.
I acknowledge that infection is always possible as a result of the obtaining of a tattoo, particularly in the event that I do not take proper care of my tattoo. I have received aftercare instructions and I agree to follow them while my tattoo is healing. I agree that any touch-up work needed, due to my own negligence, will be done at my own expense.
I realize that variations in colour and design may exist between any tattoo as selected by me and as ultimately applied to my body. I understand that if my skin colour is dark, the colours will not appear as bright as they do on light skin.
Neither the Artist nor the Tattoo Studio is responsible for the meaning or spelling of the tattoo.
I have fully read this agreement, understand and agree to be bound to it.
For everyones protection the following measure's MUST be followed until further notice. Please check each statement as you agree.

The one rule we cannot stress enough, is if you have been in contact with ANYONE with covid, or are experiencing ANY illness symptoms, it is for not only the safety of our staff and community, but the peace of mind of our staff and other guests, that you refrain from booking an appointment at this time. Please respect us!
When you enter our studio, use the provided hand sanitizer, and stay 2 meters apart from every other guest during waiting and check-out.
It is mandatory that you wear the mask provided to you for the duration of your appointment. Do not touch your face during your appointment. If you catch yourself touching your face, coughing, or sneezing, inform your artist, and go wash your hands thoroughly- immediately.
Do not bring a plus 1 to your appointment, both children or adults, as we have a 5 person maximum capacity.
We ask that you wait outside or in your vehicle and call when you arrive to our studio.
We are trying to minimize the amount of cash we are taking at this time - we do accept debit, visa, and mastercard as payment.
Every staff member will have an hour between appointments to fully sanitize all tools and workspace for your arrival, and we will be regularly sanitizing all surfaces in-between clients.
If a staff member is feeling under the weather for ANY reason, they will be sent home for 14 days, and you may be asked to reschedule your appointment.

In the event of a "second wave", or a staff member is sick, or we are asked to close down again, please be advised that your appointment may need to be put on hold at a moments notice, as the situation with COVID is changing daily. We thank you for your continued support!
